All-on-4 (and its variation All-on-X, which uses 5 or 6 implants) is a full-arch implant solution that replaces an entire row of teeth – upper, lower, or both – using a minimal number of strategically placed implants. Two implants are placed vertically at the front of the jaw; two rear implants are angled at up to 45 degrees to maximize contact with available bone and avoid critical anatomy like the sinus cavity.

The Procedure Step by Step

Using specialized software, the implant positions are planned virtually before a single incision is made. A 3D-printed surgical guide is fabricated that will physically direct the drill to the planned positions during surgery — eliminating freehand guesswork.

Any remaining failing teeth are extracted, the implants are placed through the surgical guide, and a pre-fabricated provisional bridge is secured to the implants. Most patients experience little to no memory of the procedure and leave with a full set of teeth the same day.

The titanium implants fuse with the jawbone over 3–6 months — a process called osseointegration. You wear the provisional bridge throughout this phase. A soft-food diet is maintained for 6–8 weeks post-surgery.

Once integration is confirmed, photogrammetry captures the exact three-dimensional position of each implant with sub-millimeter accuracy — data that is sent to the lab to fabricate your final prosthesis with a guaranteed passive fit.
